2019 PIT Count Survey
The Balance of State Continuum of Care uses an web-based platform to collect survey data. The online survey is accessed at https://icamissouri.formstack.com/forms/bos_street_2019. The online survey is administered by the Institute for Community Alliances. Contact mohmis@icalliances.org or your HMIS System Administrator if you experience technical difficulties with the online survey.
Communities that cannot access the online survey during the count may use paper forms. However, volunteers and county leads must enter all paper forms will need into the online survey platform after the count. Separate paper surveys are administered for youth households and adult or family households.
